Below is a list of appointments offered. Please read this information carefully. Click the button under the appointment type to be taken to my scheduling page. Select the appointment day, time, and enter your contact information to begin the scheduling process. For initial evaluations, an intake demographic form will be e-mailed to you for completion.

Initial Evaluation

These appointments are 60 - 90 minutes. The initial evaluation is our first appointment to initiate treatment. Please bring any pertinent previous treatment records, a list of previous medication(s) tried, and complete intake forms listed here. During the appointment I will be asking about current symptoms, taking a careful history, and discussing potential treatment course.

Follow Up Appointment

These appointments are 30 minutes. They are intended for initiating treatments, evaluating the efficacy of treatments, and managing treatment. Information to be mindful of when discussing treatment are timeline of symptom presentation, intensity of symptom expression, and to what degree they impact your daily life experiences.

Appointment reimbursement:

At this time I am not taking any commercial insurance. I will be considered an ‘out of network’ provider. This may help you receive reimbursement for a portion of the appointment fee. I can supply the paperwork necessary (usually called a superbill) for you to submit an out of network claim to your insurance company.
